How To Make a Referral

We accept referrals to our services for people living with a physical and/or sensory disability between the ages of 18 and 65.

Referrals must be made by a healthcare professional, for example a GP, Case Manager, hospital or community medical professional, or HSE disability service. We also accept referrals from other Community Disability Service Providers for example Acquired Brain Injury Ireland (ABI) or the Irish Wheelchair Association.

Making a Referral to Our Services

To refer someone to our services, download our referral form at the link below. Complete the form and return it to us. Our address is listed at the bottom of the page.

You will also find a ‘Consent for Release of Information Form’ at the end of the referral document. This must be signed by the person with physical and/or sensory disability and returned to us as part of the referral. Please note that we cannot process referrals without a completed ‘Consent to Release of Information Form’.

Forms can be sent to by post or scanned and emailed to the relevant address.

What Happens When You Make a Referral?

All of the referrals we receive are treated with the utmost respect and confidentiality.

Once the referral form is received, we will request information from the other health professionals referenced.

We will then arrange an initial assessment meeting to discuss your current support needs and rehabilitation goals.

If we cannot immediately provide the support services you need, you will be placed on a waiting list.

In some cases, we may not be the most appropriate service for your needs. In these cases, the person making the referral will be notified of this decision.
